The HCMG Model

Recovery from a motor vehicle collision rarely involves a single body system. Soft-tissue injury, joint dysfunction, headache, sleep disturbance, mood changes, and cognitive symptoms often present together — and resolving them requires coordinated input from multiple regulated professions.

Health Care Medical Group was established to provide that coordination in a single clinical environment. Rather than referring patients across multiple unconnected providers, our team meets, plans, and documents care collaboratively, with a shared electronic record and unified communication with funders and referring professionals.

Our Clinical Principles

  • Regulated practice. Every treating clinician is registered with the appropriate Ontario regulatory college and practises within their professional scope.
  • Evidence-informed treatment. Care plans draw on current clinical practice guidelines and peer-reviewed research, modified for individual patient circumstances.
  • Functional outcomes. Treatment plans are tied to measurable functional goals — return to work, return to driving, return to daily activities — not symptom complaints alone.
  • Defensible documentation. Assessment reports, progress notes, and discharge summaries are prepared to a standard that supports both clinical decision-making and any subsequent review by insurers, tribunals, or the courts.
  • Patient-centred communication. Patients receive clear explanations of their diagnosis, treatment options, and expected timelines, and are full participants in care planning.

Our Role in Ontario's Accident Benefits System

HCMG operates as both a treating clinic and a Structured Multidisciplinary Assessment Centre. We provide:

  • Treatment under the Statutory Accident Benefits Schedule (SABS), including treatment within the Minor Injury Guideline (MIG) and treatment plans submitted via OCF-18.
  • Insurer Examinations and Section 44 assessments.
  • Catastrophic Impairment determinations.
  • Functional Abilities Evaluations and Disability Tax Credit assessments.
  • Independent Medical Examinations for legal and insurance proceedings.
