Auto Insurance (Statutory Accident Benefits)
HCMG bills directly to the auto insurer for SABS-funded patients in most cases. Please provide: insurer name and policy number, claim number, adjuster name and contact information, and date of accident.
Extended Health Insurance
We are able to direct-bill to most major extended health insurers for physiotherapy, chiropractic, massage therapy, acupuncture, and psychology services. Please bring your benefits card to your first visit.
WSIB
Workplace-related injuries may be covered under WSIB. Approval and a claim number are required prior to commencement of treatment.
Private Pay
For patients without insurance coverage, services are billed at our published private-pay rates. Receipts are issued for income tax and personal insurance purposes.
Cancellation Policy
We require 24 hours’ notice for appointment cancellations. Late cancellations and no-shows may be subject to a fee, which is generally not reimbursable through insurance.
Affordable Physiotherapy Options
Special reduced rates are available for seniors, students, children & youth, and individuals with limited income.
Our reduced rates are designed to be comparable to rates associated with OHIP-funded physiotherapy services.
Please note: Our clinic is not an OHIP-funded physiotherapy clinic. Reduced-rate services are offered privately by our clinic and are not billed to OHIP.

All HCMG clinicians and providers are registered in good standing with their respective Ontario regulatory college. Patient information is handled in accordance with the Personal Health Information Protection Act (PHIPA).
