Orthopaedic Assessment

Assessment of accident-related musculoskeletal injuries, associated impairment and functional limitations, causation, treatment needs, prognosis, and recovery.

Psychological Assessments

Assessment of psychological injury, mood, trauma, and cognitive-emotional functioning following an accident.

Psychiatric Assessment

Assessment of accident-related psychiatric symptoms and disorders, associated impairment and disability, functional impact, treatment needs, prognosis, and relationship to the accident, where clinically indicated.

Chronic Pain Assessment

Multidisciplinary assessment of persistent and chronic pain, including physical, functional, psychological and psychosocial factors, associated impairment and disability, treatment needs, prognosis, and impact on activities of daily living and overall functioning.

Concussion Assessment

Assessment of persistent post-concussion symptoms and associated physical, cognitive, functional, vestibular, and psychological impairments following a motor vehicle collision.

Vestibular & Balance Assessment

Assessment of dizziness, vertigo, balance disturbance, gait instability, and related functional limitations following concussion, head injury, or other accident-related conditions.

Neurological / Neuro-Musculoskeletal Assessments

Neurology and physiatry assessment of neurological and neuromusculoskeletal impairment.

Neuropsychological Assessment

Assessment of cognitive functioning and neurobehavioral impairment where indicated following concussion, traumatic brain injury, or other neurological injury.

Speech-Language & Cognitive-Communication Assessment

Assessment of functional abilities, activities of daily living, instrumental activities, home and community function, work-related function, and rehabilitation needs following injury.

Form 1 Attendant Care Needs Assessment
