Case Manager — Complex & Catastrophic Injury

Coordinated case management for individuals with complex injuries, including catastrophic impairment cases.

Case management at HCMG supports individuals living with complex injuries, including catastrophic impairment (CAT) cases. A case manager coordinates rehabilitation planning, communication among providers, care coordination, and access to appropriate services so that care stays organized and connected over time.

Case management is a coordination and support service. It is distinct from an independent catastrophic impairment determination, which is a separate clinical and statutory evaluation completed by qualified assessors. Our case managers do not conduct those determinations for the clients they coordinate care for.

Involvement is arranged with the client’s consent, and information is shared with treating professionals, insurers, and legal representatives only where authorized.

What Case Management Supports

  • •Complex and catastrophic injury case coordination
  • •Multidisciplinary rehabilitation planning
  • •Coordination with treating professionals, insurers, and legal representatives where authorized
  • •Monitoring rehabilitation progress and functional needs
  • •Assistance with community resources and support services
  • •Coordination of attendant care and home support when appropriate

Our Clinical Approach

Case management begins with a review of the individual’s injury, current providers, rehabilitation goals, and existing funding pathways. From there, a case manager builds and maintains a coordinated plan, facilitates communication between everyone involved, and identifies gaps in care or access.

Support may include:

  • •Rehabilitation planning — a coordinated, goal-based plan across disciplines
  • •Provider coordination — communication among treating professionals, insurers, and legal representatives where authorized
  • •Progress monitoring — ongoing review of rehabilitation progress and changing functional needs
  • •Community resources — assistance identifying and accessing appropriate support services
  • •Attendant and home support — coordination of care supports where appropriate

Case Management in Motor Vehicle Accident Recovery

Complex collision injuries often involve multiple providers, funding submissions, and overlapping appointments. Case management helps keep those elements coordinated, and can support communication with insurers and legal representatives where the client has authorized it.

Where a catastrophic impairment determination is being considered, that determination is completed independently of the case manager coordinating care.
